Many serious accidents have occurred as a result of an equipment started running when it was supposed to be completely off to prevent repairs or maintenance. These accidents are usually caused by the energy being not released properly. Lock-out tag-out (LOTO) systems can prevent these accidents by making sure that machines are completely off until the person who is authorized releases them.
The LOTO system is made up of a variety of locks, tags and other devices that isolate, secure or shut off a machine from electricity. All "affected", "authorized" and "qualified" personnel must be instructed on the use of lockout and tagout devices. These devices include but not only chains, locks padlocks and wedges, adapter pins, and self-locking fasteners. Each device is identified and labelled, including the date and time at which it was used along with the name and address of the person authorized, and the purpose of the lockout/tagout (repairs maintenance, repairs or repairs, etc.). It must be clear that these devices can't be removed until an entire LOTO procedure is followed.
